The point-of-care ultrasonographic exam is most often performed to answer a binary question that will make or break the working clinical diagnosis made by the treating physician while seeing the patient. Unlike the traditional diagnostic ultrasound exam performed in the radiology department by an ultrasound technician and interpreted by a radiologist sometime in the future, the point-of-care ultrasound differs in fundamental ways: the point-of-care ultrasound answers the question of whether the patient has a pathologic condition or not; the point-of-service ultrasound exam is most commonly performed by the physician who is actually treating the patient who is armed with up close and personal information obtained from a history and physical examination rather than by an ultrasound technician; the treating physician often has specialized clinical knowledge of the common and uncommon conditions encountered in their daily clinical practice that may be utilized to help guide the targeted point-of-care ultrasound exam. Waldman contends it is time to embrace the paradigm shift toward point-of-care ultrasonography in the care of patients.
