This study examines the clinical and radiographic outcomes of the Ponseti method in the treatment of equinus varus clubfoot (EVCF) with a follow-up period of more than 4 years. This was a prospective study involving 28 children (42 cases of severe VEF), with regular follow-up every 3 months. Recurrences were treated with a new series of casts, anterior hemitibial transfer, or posteromedial surgical release. At the final follow-up, outcomes were analyzed based on age at treatment onset, initial severity, compliance with orthotic use, and associated malformations. The results were very encouraging. Initial correction was achieved in all cases. Recurrence was observed in 10 children (17 feet). Poor compliance with orthotic use was the main factor contributing to recurrence. The results were excellent or good in 26 children. Only 2 children had scores of 11 and 12, respectively. Analysis of the results showed a significant correlation between the occurrence of recurrence and the final outcome.
