Sanderson, a consultant who writes about person-centered thinking, planning, community building, and Individual Service Funds, and Lepkowsky, a consultant and trainer on person-centered working in the human services, describe how to develop person-centered teams in health and social care using person-centered practices. They describe how to create profiles and plans for each team member, and outline each element of these teams: their purpose, the people involved and how they support each other, successful performance, the process of working together and delivering success, and measuring progress and improving.
