Sztefko (clinical biochemistry, Jagiellonian U., Poland) details concepts of basic immunochemistry and knowledge needed by physicians and laboratory professionals about immunoassay results due to interference from sample constituents, to protect patients from misdiagnosis and mistreatment caused by erroneous lab results. She describes the basic principles of immunoassay technique, the assay standardization and calibration, and the main problems connected with reference intervals for the analytes most frequently measured by immunochemistry. She then covers the preanalytical phase of measurement, including factors influencing test results, such as sampling the blood, use of anticoagulants and other tube additives, and typical interference like lipemia, hyperbilirubinemia, and paraproteinemia; natural antibodies in serum or plasma samples; interference in relation to assay format and methods for detection, elimination, and reduction; and recommendations for improving patient safety. Seven examples of these theoretical problems are described in the second part: parathyroid hormone, human chorionic gonadotropin, troponin, aldosterone and proteolytic renin activity, thyroglobulin, prolactin, and thyroid function tests.
