前言：什麼是醫療品質管理「醫療品質管理」是什麼？為什麼醫療需要「品質管理」？休哈特(Shewhart) 指出，品質管理的基礎在於製造過程中以科學的管理方式提升產品品質。以豐田生產系統為例，透過品質管理作業，能確保各階段的中間產品及最終製造出的汽車能維持高品質的水準。那麼，醫療呢？根據病人的需求，醫療提供者進行診斷和治療（相當於產品的提供），協助病人回歸社區生活並從事相關活動。若以休哈特製作程序(manufacturing process) 的觀點，將門診服務拆解為報到、診斷、檢查、治療等程序，這些程序產出給病人的產品( 亦稱為製程間庫存，inter-process inventory. ) 包括病歷、檢查單、檢查結果、診斷、手術/ 給藥，當上述的生產過程發生缺失，最終的產品品質也會受到影響；也就是說，過程的疏失最終會影響治療結果（如康復出院或持續與疾病共處）。這樣看來，豐田生產系統與醫療作業並無太大差異，惟醫療是種更複雜、多變化、擁有不確定因素的系統，包含治療、檢查、護理照護、供膳、社區協作和醫療相關事務等，因此，豐田生產系統如果需要品質管理，那麼更複雜的醫療系統會更需要品質管理。當思考到這一點的時候，我們就可以理解品質管理對於醫療的必要性並起而行之，雖然剛開始推動會遭遇許多困難，員工也會因為改變感到焦慮，但不必擔心，品質管理有許多不同類型的手法可供選擇，例如品管圈活動中最常見的「QC story」。另外，在進行品管活動時，建議您無論成效如何，重要的是踏出第一步，不斷去嘗試和挑戰Try it!
