序言 2018 年諾貝爾醫學獎，頒給美日科學家詹姆斯‧艾利森和本庶佑，表彰其在免疫療法的貢獻，為癌症治療掀開嶄新的一頁。隨著癌症免疫療法的加入，加上各式免疫生物製劑的持續進展，免疫學在臨床的應用也越趨重要。這也代表著，基礎醫學中的免疫學，與臨床醫學的關聯更緊密。 顯然，免疫學考題只會越來越熱門。幸運的是，免疫學的考題並不難掌握，而且暑假考題常常會和寒假考題的主題重複。相較於微生物學比較瑣碎的記憶（常需編各種口訣才能記起來），免疫學算是相當有邏輯與條理的。 在準備免疫學的時候，我會先畫出一張機制圖，串連抗原如何被捕捉、如何呈現給 T 細胞，T 細胞與 B 細胞又是如何溝通？本書的第五章—抗原呈現的機制圖—正是我自己讀免疫學時，畫的第一張藍圖。心中有個地圖，很多細節就可以按圖索驥。遇到新資訊時，也可以將其依序放進圖中。例如：讀到免疫療法時，看到 CTLA-4 和 B7，知道他們是 T 細胞與抗原呈現細胞互動時的次級訊號，就可以把他們放在圖中「T 細胞被抗原呈現細胞活化」的這一塊。關於 B 細胞與 T 細胞的發育，由於紙張範圍有限，很難將我心中的圖一次畫出來給大家看，故拆成若干章節。在此，我也想請大家記得它是一個連續的流程，可以先畫出發育三階段的大綱（例如：負選擇正選擇、被抗原呈現細胞活化、Th1/Th2 的分化等），再將各章節的資訊放進去。運用邏輯思維去整合內容，記憶會更長遠。 免疫學的後半章節，則偏向臨床的應用，此部分的考題向來繁雜，但各項疾病都可以歸納成幾個大類別。我自己習慣以表格來整理這些繁雜的資訊，例如：最為瑣碎的免疫缺陷疾病，還是可以大致分成 B 細胞、T 細胞、補體出問題等大分類。若能掌握這個疾病屬於哪一類，大概就問心無愧了！剩下太細的就交給運氣吧，畢竟二八法則還是有它的價值的。 許多人在準備第一階段國考（基礎醫學）時，常會聽聞這些以後臨床都用不到，為什麼還要記這些？事實上，以微生物學為例，臨床上每天最常遇到的就是各式感染症，以及如何正確選用抗生素。而在免疫學部分，我認為學好免疫，對於未來臨床研究至關重要。從內科、兒科，到眼科、皮膚科等，都可能遇到自體免疫相關的疾病，且常常是當今炙手可熱的研究主軸。最實際的例子，就是當別人在談論人體如何抵禦病毒、疫苗如何讓人聽
