引言 2016年衛生福利部推出「醫病共享決策平台」，積極推廣醫病共享決策（Shared Decision Making；簡稱 SDM），目的是讓醫療人員和病人在進行醫療決策前，能夠透過醫病的相互尊重與溝通，共同享有現有的實證醫學成果，造福患者。SDM推出後，各醫院踴躍配合執行，已取得具體成果，提升了臨床診療品質。SDM是以病人為中心的臨床醫療執行過程，兼具知識、溝通和尊重三大元素。透過促進醫師和病人雙方共同參與，由醫師提出各種不同處置之實證資料，病人則提出個人的喜好與價值觀，彼此交換資訊討論，共同達成最佳可行之治療選項。SDM的落實，除了以病人為中心，以實證醫學為依據，還必須推動明智選擇 (Choosing Wisely)。欠缺明智選擇的SDM，有美中不足之感。「明智選擇」(Choosing Wisely)是美國內科醫學委員會(The American Board of Internal Medicine, ABIM) 2012年發起的一項跨專業運動，目的是找出各個專業領域對於患者，尤其是無症狀民眾，沒有必要甚至可能是有害的檢驗、檢查、手術或治療的清單。每個專業領域通常列出了5項或10項被過度使用的檢驗、檢查或治療。這些專業學會每年會審查和更新清單，確保其反映最佳的實證醫學證據。近幾年來，SDM偏重在臨床處置，相對忽略了檢驗項目的明智選擇 (choosing Wisely)。為了使SDM更趨完善，台灣醫學實驗室管理學會特蒐集相關文獻，並參考多個國家的Choosing Wisely建議，例如美國ASCP-Choosing Wisely、澳洲、加拿大和英國Choosing Wisely的建議，編著本書有關檢驗項目使用管理 (Laboratory stewardship)之Choosing Wisely建議供醫病雙方參考。期望醫病雙方在溝通和選擇檢驗項目時，彼此能有正確的認知與共識。並做為諮詢醫檢師的培訓教材。本書內容是遵循國際上Choosing wisely（明智選擇）的精神和共識，聚焦於不該做的檢查。所有不該做的檢查肇因於不適當的檢驗申請 (inappropriate testing order)，不適當的檢驗申請包括濫用(misuse)、過度使用(overuse)、未充分使用 (underuse)和不當的複檢 (inappropriate retesti
