臺灣的醫療保健屬世界上高質量且國際認可的醫療保健系統，但衛生主管必須從所有疾病負擔的角度來看待整體國民健康，而不僅僅是醫療狀況本身。健康維護在醫療技術高速發展的時代依然不足，三段五級介入需求不斷增加，最重要的還是初級預防。我們以缺血性心腦血管疾病為研究對象，力圖梳理兩者的危險因素疾病及其相關流行病學情況、更關注長期照護制度與失能調整後人年（disability-adjusted life years、DALY）和生活品質調整後人年（quality-adjusted life years、QALY）的討論，以充分呈現所謂的整體疾病負擔是利益相關者應該考慮的重點、並成為年報中呈現的標準化數據。臺灣已步入高齡社會，除了癌症之外十大死因前五名涵蓋了心臟病及腦血管疾病，二者有越來越年輕化之趨勢，因此我們結合公共衛生疾病負擔專家、臨床教授及醫師，針對心臟疾病及腦血管疾病進行此計畫；主要針對心肌梗塞及腦中風，整理並比較醫療成本及不健康平均餘命、疾病負擔進而提出相關預防對策及建言。從衛生福利部2021年報1995到2019年發現心肌梗塞及腦中風死亡率，因醫療技術之進步都有下降的趨勢，另外從2012到2019年單次醫療費用也呈現下降的趨勢，而終身醫療費用在腦中風是呈現上升。使用長照資源的量與金額，心肌梗塞高於腦中風，但腦中風在使用嚴重程度之支持系統比例較多。兩個疾病之危險因子基本上相同，心房顫動是心臟疾病卻是腦中風重要之危險因子，我們發現心房顫動在分析2012-2019年健保資料後顯示服用抗凝血藥物治療者追蹤發現有17.9%發生中風，而未治療者為13.3%，但治療效果是否比沒治療差必須進一步完整研究，調整疾病嚴重度及共病狀態和治療配合度等才能確定真正情形。因此在目前初段預防針對相關危險因子包括心房顫動的治療，所進行之對策應持續強化； 早期診斷心肌梗塞已具有效的工具，但腦中風只有頸動脈超音波及穿顱都卜勒超音波檢查（顱內血管超音波檢查），如何有效開發次段預防技術及工具是重要課題；目前在心肌梗塞的緊急救治上已有結合救護車上無線傳輸12導程心電圖醫院心導管式預為準備的救護體系，在整體救治效率及存活率上大幅提升，反觀在腦中風末段預防的緊急醫療上囿於設備限制亟待開發，另外在橫向聯繫及資訊傳輸仍有強化之空間，以建立更具效率的救護能力。
